Sangre de Cristo Community Care provides award-winning home health care to families in Trinidad, Walsenburg, Cañon City, Pueblo West, and La Junta, offering skilled nursing, CNA support, physical therapy, and certified wound care through a compassionate, comprehensive team.

Palliative Care

An innovative health care service that provides you with an extra layer of coverage and peace of mind for the stresses that may come with a serious illness.

Home Health Care

Dementia Care

This program is available to Medicare beneficiaries throughout Southern Colorado, and individuals may self-refer or have their healthcare provider send a referral for an evaluation to determine eligibility, offering benefits such as a dedicated care navigator, 24/7 nurse line access, caregiver training and support, assistance connecting to community-based services, and respite care for those who qualify.

Volunteer Support

Our trained volunteers bring their unique talents to hospice care — whether sewing comforting blankets, sharing music, offering administrative help, or providing companionship and support to patients and families. Whatever your talent is, you can make a meaningful difference as a hospice volunteer.

Grief Support

Grief counseling and support for family members and significant others for a minimum of 13 months (more, if needed) following a loss.

Additional Services

Social Services

Psychosocial support for patients and families, as well as for the nursing home staff and care givers.

Medical Coordination

Coordination of medical equipment and medications related to the terminal illness for comfort and symptom management.

Spiritual Care

Spiritual support, as requested, for patients and families. Our team of chaplains coordinates support in accordance with individual beliefs and preferences.

Therapies

Physical, occupational, nutrition, music, and other therapies as indicated in the plan of care.
