As a nonprofit organization, Sangre de Cristo Community Care has remained dedicated to providing proven, compassionate care to patients and families throughout Southern Colorado for nearly four decades. We proudly serve our communities with a full continuum of support — including hospice, palliative care, home health, dementia care, and comprehensive bereavement services.
Hospice Care
Choosing where to spend one’s final days is deeply meaningful, and Sangre de Cristo Hospice provides an integrated, team of medical, emotional, and spiritual professionals to ensure compassionate care wherever a patient calls home.
Palliative Care
Palliative care enhances a person’s care by focusing on symptom management for people with serious illness.
Home Health Care
Sangre de Cristo Community Care provides award-winning home health care to families in Trinidad, Walsenburg, Cañon City, Pueblo West, and La Junta, offering skilled nursing, CNA support, and physical therapy, through a compassionate, comprehensive team.
Serving Southeast Colorado
As the only nonprofit hospice in Pueblo, we have expanded our reach to La Junta, Cañon City, Trinidad, and Walsenburg to ensure rural communities receive the care and dignity they deserve. Find Grief Support
Our team of grief educators, social workers and trained volunteers support individuals of all ages who are grieving. They help families from those Sangre de Cristo Community Care has cared for and community members with grief from many different situations in life.

Refer a Patient
Anyone can refer a patient to hospice, and early referrals greatly improve quality of life by allowing symptoms to be anticipated, managed, and prevented before they become severe. When hospice is involved sooner, patients and families receive more comprehensive support and can often avoid unnecessary hospitalizations or emergency visits.
