While many Sangre de Cristo Hospice & Palliative Care patients receive care in their homes, hospice care is available in assisted living, nursing homes, and group homes to allow those residents in advanced stages of illness to remain in familiar and supportive surroundings while receiving hospice and palliative care. Our team works to enhance the care of the assisted living and nursing home team with the special care, emotional support, and spiritual guidance that residents and their families need during this distressing time.
