Many of our patients prefer to receive hospice care at home. If this is your preference, we’ll do whatever possible to keep you in your home or with your family and friends. We understand there is no substitute for being in a familiar environment surrounded by those you love.
Our experienced team will visit you to determine whether hospice is right for you. If it is, members of your hospice care team will make regular visits to your home. You and your loved ones can call whenever there is a problem or if you have a question between scheduled visits. Many immediate concerns can be answered over the phone, but, if not, a team member will visit as soon as possible.
Remote Patient Monitoring
From 24/7 care coordination to bereavement care, Sangre de Cristo Community Cares continues to deliver the services that patients and families rely on, even when we are unable to be physically present. We are employing the use of Telehealth technology when appropriate, including Telehealth admissions for hospice-eligible patients.
Our care team members continue to consult with families, helping them work through providing day-to-day care for their loved one. Our bereavement services and community support groups provide assistance for grief that may be heightened by an inability to visit or difficulty finding closure.
