(Pueblo, Colo.) — Sangre de Cristo Community Care has proudly earned Level 5 status from The National Hospice and Palliative Care Organization’s We Honor Veterans (WHV) program – the highest attainable level in this program that requires a great deal of dedication to provide Veterans with compassionate care that is specific to their needs and goals.
Sangre de Cristo Community Care is the only We Are Veterans Level 5 provider serving Southern Colorado. “Many of our most memorable moments are those where homebound Veteran pinning’s are done and personal stories are shared,” said Anisha Lucero, MSW, LCSW, vice president of family services at Sangre de Cristo Community Care. “We at Sangre De Cristo Community Care are honored to serve our Veteran communities throughout Southern Colorado and look forward to serving them for years to come through our We Honor Veterans Level Five partnership.”
Earning Level 5 status ensures that partners place a greater emphasis on staff education and caring for Vietnam- era and combat Veterans.
NHPCO and the Department of Veterans Affairs launched We Honor Veterans in 2010 to address the growing need for Veteran-centered care, especially as Veterans are aging and need access to palliative and hospice care.
As a WHV partner, Sangre de Cristo Community Care addresses the unique health care needs of Veterans, both physical and emotional, including the psychological toll of war and how it impacts their end-of-life journey.
