Admission to home health requires a physician certification that home health services are medically necessary and require the skill and knowledge of a nurse, physical therapist, occupational therapist, or speech therapist. We are concerned with the total care of our patients and their satisfaction related to our care. Sangre de Cristo Community Care serves a variety of patients, including pediatric patients and those with developmental disabilities, and may include any or all of the disciplines outlined below. Our licensed staff will develop an individualized care plan and coordinate services that are reasonable and necessary to address your illness or injury and help you reach your maximum recovery to health. You will be asked to participate in planning your care and to identify your individual goals. Medicare, Medicaid, and most Commercial Insurances provide benefits for services in your home and our staff will advise you of the admission and coverage criteria of your specific insurance.
