Sangre de Cristo Community Care has been providing home health services to the communities of southern and eastern Colorado since 2018. In 2021, Sangre was named an HHCAHPS Honors Elite recipient by HEALTHCAREfirst. We are committed to continuing to provide excellent care in our communities in Trinidad, Walsenburg, Cañon City, and La Junta.
