Sangre de Cristo Community Care is excited to add this additional supportive program to our continuum of care. This program is available to Medicare beneficiaries throughout Southern Colorado. Individuals may self-refer or have their healthcare provider send a referral for evaluation to see if they qualify for this new supportive program.

- Caregiver training & support
- Assistance with connecting to community-based services
- Respite care for those who qualify
To qualify for this program, an individual must have:
- Traditional Medicare
- Dementia Diagnosis
- Not be a resident of a nursing home, or enrolled in a PACE program
