Hospice Care — Regular visits by registered hospice nurses with specialized training and expertise in pain and symptom management.
Palliative Care — An innovative health care service that provides you with an extra layer of coverage and peace of mind for the stresses that may come with a serious illness.
Volunteer Support — Trained volunteers who provide companionship, assistance, and support through the entire care process.
Grief Support — Grief counseling and support for family members and significant others for a minimum of 13 months (more, if needed) following a loss.
Social Services — Psychosocial support for patients and families, as well as for the nursing home staff and care givers.
Spiritual Care — Spiritual support, as requested, for patients and families. Our team of chaplains coordinates support in accordance with individual beliefs and preferences.
Medical Coordination — Coordination of medical equipment and medications related to the terminal illness for comfort and symptom management.
Therapies — Physical, occupational, nutrition, music, and other therapies as indicated in the plan of care.
